Dr. Anna Ebey, PT, DPT, CSCS

Owner & Physical Therapist

Dr. Anna Ebey is the owner of Associated Rehabilitation and brings nearly two decades of clinical experience to her practice. She earned both her undergraduate degree in Biomedical Sciences and her Doctorate in Physical Therapy from Marquette University, later achieving certification as a Strength and Conditioning Specialist (CSCS). With a broad clinical background, Anna enjoys treating a wide range of conditions—from orthopedic and post-operative care to neurological rehab and athletic performance. She has a special interest in vestibular therapy and is passionate about helping patients regain confidence and function after experiencing vertigo or balance disorders. For the past five years, Anna has led the clinic with a patient-first philosophy, blending science-based care with a personal touch to help individuals of all ages move better, feel stronger, and return to the life they love.

Dr. Brett Taylor, OTD, OTR/L, Cert. DN

Occupational Therapist (Lead Therapist)

Dr. Brett Taylor is a licensed occupational therapist with a clinical focus in sports therapy, upper extremity rehabilitation and hand therapy. Brett graduated with his Bachelor’s degree from the University of Arizona in 2019 and went on to earn his Doctorate in Occupational Therapy (OTD) from the University of St. Augustine for Health Sciences in 2022. Brett’s passion lies in helping patients regain optimal function and independence following injury, surgery, or chronic conditions affecting the hand, wrist, elbow, and shoulder. With specialized training in upper extremity rehabilitation, he blends evidence-based techniques with patient-centered care to support recovery, reduce pain, and restore movement. He is also certified in dry needling (Cert. DN), which he integrates into treatment plans when appropriate to assist with muscle tension, pain management, and improved mobility. His clinical philosophy emphasizes active collaboration with patients — ensuring each person feels heard, supported, and empowered in their healing journey.

Stephanie Enders, PT, MPT

Physical Therapist

Stephanie Enders brings over 30 years of experience as a licensed physical therapist, specializing in general orthopedics and neurological rehabilitation. She earned her Master’s in Physical Therapy from Northern Arizona University and has since dedicated her career to helping patients restore movement, reduce pain, and improve overall quality of life. Stephanie’s compassionate, hands-on approach is rooted in evidence-based practices and tailored to meet the unique needs of each individual she treats. Whether working with post-operative patients, those managing chronic conditions, or individuals recovering from neurological events, Stephanie is committed to delivering personalized care that supports lasting recovery.

Dr. Yolanda Benitez, PT, DPT, CAFS, CIDN

Physical Therapist

With a decade of experience in the field, Yolanda brings a holistic and movement-based approach to physical therapy that is grounded in both science and compassion. She earned her Doctorate in Physical Therapy in 2015 and has since specialized in neuro rehab, orthopedics, and sports injury recovery across a variety of clinical settings. Certified in Applied Functional Science, Yolanda focuses on how the body moves in real-life situations, rather than treating isolated symptoms or body parts. Yolanda is also Certified in Dry Needling (CIDN) and works to integrate a range of manual therapies into her sessions, including instrument-assisted soft tissue mobilization (IASTM), cupping, and other evidence-based techniques. Her comprehensive approach is designed to optimize recovery, reduce pain, and support long-term wellness. Yolanda is passionate about empowering her patients and takes pride in tailoring each treatment plan to the individual’s unique needs, goals, and lifestyle. Her goal is to help every patient return not just to activity, but to life.

Tara White, PTA 

Physical Therapist Assistant

Tara White is a licensed Physical Therapist Assistant who graduated from Pima Medical Institute in 2024. She supports therapists in delivering individualized plans of care, with a focus on orthopedic and neurological rehabilitation. Tara brings a unique perspective to patient care through her background in yoga and personal training, blending mindful movement with functional therapy techniques. Her holistic approach promotes strength, balance, and recovery, helping patients achieve their goals in a supportive and empowering environment.

Jonathan Amaya, PTA

Physical Therapist Assistant

​Jonathan Amaya is a licensed Physical Therapist Assistant who graduated from Carrington College in 2022. He focuses on general orthopedics and neurological rehabilitation, following individualized treatment plans under the direction of licensed physical therapists. Jonathan is passionate about helping patients regain function and improve their quality of life through attentive, hands-on care. As a bilingual clinician, he is dedicated to creating a comfortable and inclusive environment for both English- and Spanish-speaking patients.

How Can We Help?

Do I need a doctor’s referral?

In Arizona, a direct access state, you can make an appointment with a physical therapist directly without a physician referral. 


This simply means that if you feel you have a problem that may benefit from the skilled services of a physical therapist, you are able to refer yourself to physical therapy for treatment. 


The immediate access to therapeutic care in cooperation and conjunction with the diagnostic assessment of the physicians and supporting medical community leads to a much higher rate of success in a timely manner for the individual seeking services.


*Keep in mind that your insurance policy may require a visit to the primary care physician first or may limit your access to preferred providers only. Medicare beneficiaries are able to go directly to physical therapists without a referral or visit to a physician.

What is your new patient procedure?

After calling to schedule an appointment, you will arrive 15 min prior to your selected appointment time to fill out evaluation paperwork. 
Please bring your insurance card, photo ID, referral if needed, and any pertinent paperwork or imaging results from your doctor. 
Please dress comfortably.
